Purchase a Broken Mask of the Hidden from either Tatsujiro the Serene or Xeib Darkskies in Lavastorm Mountains for 500 Radiant Crystals or 500 Ebon Crystals. (No dialogues for this step.)
Both Yar`Lir the Matriarch and Vishimtar will drop an eye every time. When you combine the two eyes in the broken mask, you create a Mask of the Hidden. Wear that when entering the upper half of The Ascent, and you will see: As you don the mask, your vision blurs for a moment. As your eyes come into focus, you see a glittering path guiding you... somewhere. This path will lead you up to the peak of The Ascent, where you will find "a mysterious chest" at +6375, +1330, +755 which contains the quest reward: This chest is on your map and will give you a find path via CTRL F. As it comes into your hands you feel the mask dissolve and slip off your face into oblivion. Also, "a mysterious chest" location will appear in your map and is findable via Map Find.
I just did this quest for a couple of my chars - you get the emote about donning the mask seeing a glittering path guiding you...somehwere - at the bottom half of the ascent before the clicky-up gate is even in view. The chest spawns immediately at this point.
You immediately get the "Mysterious forces prevent you from finding your destination easily" failure and no path appears. Continuing up the ascent, taking the mask off/on does nothing to re-establish the glittering path.

what im reading is i cant lose the item? someone told me if i die or camp i lose the mask and have to redo the quest is this true
This is not true.
The only items you will lose when camping out are those with the TEMPORARY flag. And even then, you must be camped out for more than 15 minutes.

I was wondering if anyone who has completed this quest could clarify something for me??? When you enter upper ascent, there is quite abit of distance to cover from the zonein to upper ascent to the summit.
In the normal version of the zone is littered with a great number of mobs one must pass in order to reach the summit. Granted, if memory serves the first half of upper ascent can usually be bypassed with simple camo, but at somewhere to 1/3 to 1/2 way up there are quite a few mobs that see thru camo.
My question is this, when you enter upper ascent with the mask on which triggers the illuminated path to the chest at the summit which contains the reward, do the mobs spawn just like usual when one enters the upper half of ascent or does it spawn with no mobs at all like it does when one zones in after obtaining a DoN raid? Many of those raids deposit you into the zone with no trash and the raid enters very close to the boss for the particular zone in question.
Basically, I'm trying to determine if I ought to assemble some friends to tag along for clearing of the trash otw to the mysterious chest or whether I can just do it solo... I have already surmised that I will retain the mask until I actually loot the reward from the chest based upo the info already provided in the quest details, which is a good thing, obviously. I am just trying to determine whether to plan for a group and time for basically clearing the upper ascent of mobs...
Any info on this point is appreciated and thanks in advance if someone is able to clarify this part of the quest for me.

You are not going into an instance. The final part of this is done in the NORMAL ascent, with all the normal mobs.

I find it very upsetting that Sony put the Druid item in the face slot, making it so that we have to decide between Dragon Frost on the Mask of Raining tears or this item. They chose possibly the worst spot they could have on a druid.

The focus is duplicated on druid gloves from anguish and the clicky is usable from inventory I believe. So it's not so bad all in all.

I think I did pretty good on my prediction...
I had figured they'd give us Alacrity of the Anuek and one of our spears as a clicky... but wow... Touch of the Devourer as a clicky and an amazing ER focus... even better than I thought!
Now to drool over it, cause I won't be seeing it any time soon, lol.

Completed, I believe Lazorus or some spelling of that on Bertox was the lucky first SK.
Your assumption on the focus and clicky are almost guaranteed to be correct, but the item is not going to be a shoulder. There are 16 classes, and 8 potential slots (non-weapon, non-armor): ear, neck, face, finger, shoulder, back, waist, ranged. Two classes per slot. Shoulder is full up, so is ranged, and finger, and mask. Smart money is on Ear, since that still has two holes left, but it could be any of the other four just as easily.
Oh, and you left off the +% mods. Probably ds, mana regen, shielding, and one of dot/spell/stunresist.
