The Ring of the Dead was the alternate reward for the Thex Mallet quest http://everquest.allakhazam.com/db/quest.html?quest=238. The other reward was the Reaper of the Dead. If only there was some way to tell by the name of the items.
The Thex mallet isn't a reward per se. Someone had to actually do the quest all the way AND turn the mallet in and people were afraid of handing in a magical weapon for an uncertain reward.
Pretty sure it was also in the Everquest quest compendium from the original Safehouse site.

Information on this is sketchy. There is a female NPC in the Necromancer Guild who will talk to you about a Ring of the Dead. I'm not sure if she is the one to talk to to get the ring. I have been told that she will recharge it if she really likes you, but won't give you the ring.
