EDIT: And yes, he spawns in place of the putrid skeleton with the rusty ******* sword underneath that tiny structure.
To spawn the Live version kill the a putrid skeleton between 5 and 8 pm game time, then set a timer for 3 hours. For the Undead version, kill the placeholder between 5 and 8 am game time, and set the timer for 3 hours.
This doesn't work any more, if it ever did. I killed all putrid skeletons around 5 PM game time, and again as soon as they respawned which was either 7 or 8 PM game time, then 2 hours and 40 minutes of real time later, undead Lord Grimrot spawned. I bet it's random whether you get live or undead.

I’m also interested in this quest. I really enjoyed the Bloodsabers quests and this one leading to Castle Mistmoore has been a bit of a puzzle for quite a while.
Before the official forum makeover (in fall 2012 I believe), there were a small number of threads on the official forums concerning Grimrot and Arlena. I just searched the archived forums but found nothing relating to either character. I find this strange but that's another issue. What I remember reading is that, at one time, Arlena was indeed found in the Castle Mistmoore zone, but was removed very early in the history of EQ. By very early I’m thinking before any of the expansions were released. But I can no longer find those posts to reference them.
In addition to the ground spawns posted by Yther, I found the following:
It may be that the quest is tricky and has not yet been completed, or that the quest is now broken as a result of Arlena being removed from the game. But I still have hope, every once and a while I’ll take a character through the zone looking for some sign or clue :)
I searched for anythign relating to Grimrot, Arlena, bones, and remains, and found nothing, other than Grimrot's drop, his heart. I will look for re-arranged letters of Arlena later. Didn't find any place that listed the Mistmoore ground spawns, but there are a bunch of them, one that I don't remember if it was known what it's for, was inside one of the rooms, just as soon as you drop down to the secret passage from the graveyard.
Since I couldn't find any items, I'm thinking it's probably a ground spawn, since there are so many Mistmoore.
Just now re-arranging letters in my head, got Near LA (or maybe LA = Library Assitant).
Sorry not much help, but maybe I gave some directions you hadn't thought of already. :-/

**** EDIT **** A quick look last night at the ground spawns I could remember in Mistmoore, found these items: Leatherbound Journal Just talks about making Ornate Chainmail Icon of Sacrament This is the one I mentioned earlier, but used in another quest Dusty Ribcage Ribcage Of Darak Lightforge, used in a Paladin quest Taste of Enticement Enc 1.5
I still believe there are some more ground spawns there. And still like Near LA (Library Assistant) as a clue, but may just be my fasciantion. :-/

I'm beginning to feel like Grimrot's undead version may just be a myth. I've been trying for his undead version for over a week, and I've only ever seen/killed the live version once. I don't even know what the undead version LOOKS like, so a lot of times, I end up clearing the area around the PH spawn just so it might help determine undead Grimrot from the other skeletons. Can anyone please post a picture of undead version, or maybe email it to me? Superstooge@comcast.net
Looks like a normal skelly. Came across the bridge from NK to do some hunting and there he was in the center of the ring - with his pet. Conned DB to a level 34 Bard.
Was running through SK today with my 56 Ranger and say old boy on track. Figured I'd take him out. It was the undead version. Piece of cake kill and dropped the Pestilence Scythe (not the quest version it seems) and "a decaying heart". I haven't found anything around about this heart. Apparently not related to the qeynos quests involving decaying sternum and such. Anybody with any clues on this?
This guy can be found in the undead camp fairly close to the entrance to NK. Zone in from NK and go south and you will run straight into the undead camp. Conned Db to my 39 Beastlord. I pulled up warder, medded to about 50% mana after fulling buffing myself and warder and went to town. He is a toughie. His HT did about a full blue to my warder in HP and he hits 30-80. Plus he does all the nasty SK spells. Strength steal, and whatever one looks like mud and makes you walk super slow. He got me down to 22% before dying and no mana left and then I still had to deal with his pet. His pet died in about 2 rounds from me and hasted warder (Yekan's Quickening). Had I prepared better and been at full mana this guy would have been much easier.
I am a gnome Cleric of the 45th season on tallon zek i camped the skelly camp for my aegis of life i have soloed the live and undead one and all i have to say is its easy as cake. For the undead one wich took 8hours to spawn just beat him down with all ur undead spells including dots like turning of the unatural. For the live one wich is a little harder i feared the pet then nuked the lord with resolution for full dmg he HT'd me. So i think buff urself wiht all u got. When he HT's u there is ur Q to dominate thats all hes got root him until one sticks then heal urself. go back fear him and then keep nuking untill hes dead. (pet stayed feared the whole time.)
I just killed Lord Grimrot today.. Had no idea what he was for as I'd never seen him there before. Conned Light Blue to me at 49, so I figured it'd be an easy kite... Wrong! this guy has insane magic resistance.. I must have tried to stick my chants on him 30 times before I have up.. not a single one took hold. I had to bust out my wristsnapper and symphonic saber and go at him melee style. I was twisting Jonathan's Provocation and three pulses of Shield of Song to lessen his blows.. Damn though.. He was consistently still doing better damage than I was and I had to run in circles at about 2 blue bubs of health just to regen a bit. Then I busted out Denon's, the bard's equivalent of harmtouch.. his pet took the full 361, Grimrot only took about 180 damage.. still evened the odds in my favor though, and I was able to kill him with a bubble of health remaining. Dropped two versions of the scythe, the quest item and a regular one which for some reason bards can equip even though we do not get 2HS skill. If you're planning on taking this guy out, be prepared for a good fight.. he tried to fear me multiple times, stuck a steal strength on me which lasted at least 15 minutes, and hit for into the 60s I think, tough little ******* for his level. Not worth the effort unless you really need that quest Scythe IMO.
I've been camping this guy for 6 days...killing the PH between 4-7am and pm. Im not there to kill him...Im doing the trumpy irontoes skull quest. Anyway, so far no luck. I actually went through the trouble of petitioning and asking if he was broke, of course that just resulted in a GM telling me "as far as I know...no". I actually had hopes they would make him spawn to make sure heh. As far as I know Im doing everything right..its just a random spawn and Ive been very unlucky. I will post again when I find out the reward for the skull quest cause nobody seems to know what it is.
*yawn* Been killing the PH putrid 4-5 times in a row, almost every time at 5 (either am or pm, not keeping track) for a few days. Neither Grimrot has spawned. Getting lots of faction with mass-killing the camp (is that the faction needed to get the quest?) also farming bone chips for faction. Sigh...so boring. At least I can powerlevel my lil guys there at the camp.

I have never done the live version, but the dead version was pretty easy for my 29 Ranger to solo. His harm touch almost got me, but with my melee and nukes of Ignite in between swings, I was able to outdamage him. I just rooted him to begin with and then meleed his pet to death (can't have him hanging around interrupting your casts), root held the till pet was dead, didn't even bother to root him again (that was a mistake). Next time I would keep him rooted and nuke him to death to avoid that Harm Touch, or maybe bow kite with SoW up (didn't have SoW at that time). He con'ed white to me at 29 though his level is given as 30, I was very close to leveling myself so that must be why. They say that the live version has good Magic Resistance, so I would be careful to make sure you have all your AC and HP buffs up and use your melee skills to beat him. Anyhow at 51 it should be very easy.
One more thing. Live Lord Grimrot does have very high MR/FR/CR (yes his all around resists are good not just MR) for his level. He is no match for lures however. If a 44+ Wizard is handy to help you, get them a sow and they can kite this guy easy.
To confirm what the previous poster stated, it's very easy to get the ph/Grimrot to spawn at night or day. Whether it's useful to do so is another question but killing him at 5PM or AM always resulted in the desired time frame for me. BTW I eventually saw undead pop at night and live pop during the day.
5pm/5am CST? PST? EST? ??? So far I've kille the undead a couple of times, no sweat for a pally or cleric, undead is the bugger I'm waiting on. Starting to believe he only pops in the morning.

You're not a noob if you subscribe to the ph timing theory. The overwhelming evidence from people who've done the camp is that, at the very least, undead Grimrot is far more likey to spawn at night than live Grimrot and vice versa. As for the possible 45 minutes variance in spawn times, if you were to plot a graph of spawn times you'd find a big cluster of points right around the 3 hour 14 minutes point. Does't have to be a guaranteed time for this theory to make sense.
I happened to find him in skele form. Just passing through, seeing what his loot was for. Got that scyther thingy and a Decaying Heart. He conned even to 31, so he can be other than 30 i gesss. He harm toched foro about 331 i think, something in the neighborhood. Low life, pathetic resistances.
