I just killed her north of Deadbone Reef. I had killed a few undead sailors a few minutes prior and killed nothing afterward. After a bit of time the sailors repopped on track, and then a few minutes later a great white shark appeared on track. In the 10 minutes after the shark showed up, nothing else did (apart from the transport ships).
After killing the shark, a goblin spirit caller came on track. The goblin spirit caller was followed by a goblin shaman.

I am convinced that the PH is a Goblin, not sure which kind.
Wanted her to spawn and have been tracking and killing nothing but goblins with my druid.
After she spawned and I located her my monk came in to finish the job.
She spawned 3 times in a row after killing various kind of Goblins.

Hit my 77 bard for up to 1706. Has no special melee abilities. Ran when below 10% health.

She drops a mask called Hag's Eye in addition to the belt. I have killed 4-5 times and seen the mask twice, so I'm not sure which is her rare and which is her common drop.

Relentless Pain is also a mana-drain DoT - fast one.
The DoT is called Relentless Pain and hit me for 790 per tick.
He has DOT 1000 per tick
