I dont under stand you kill one mob and this pops?
and further if you doing teir 2 quest you have to wait to kill again or does all this drop at once?

There's an event in anguish that involves killing 3 mobs (the Tormentors) at once. Once all 3 are dead, this chest pops with loot in it. (2 regular items, 1 tier 2 item, epic 2.0 orb, one anguish backflag Seal, and 2 ancient runes). Repop time on the event is 5.5 days iirc.
