Afaik, all the 50 Required level Flawed stuff is from starter characters on the 51/50 server that is no more. You probably want this https://everquest.allakhazam.com/db/item.html?item=84172 Flawed Adept's Belt. However, it gives no source info either, but my guess since all defiant waist, back, and shoulder items are easiest to obtain from a level appropriate Franklin Teek hotzone quest, that he would be the best place to start. Also, since they're attuneable might try the bazaar.
