With the first gem inserted the buff gives 100hp + infravision.
Each additional gem inserted gives an extra 100hp + a certain amt of mana + some other benefits.
And no, at this time it will not stack with the Naggy Familiar. The highest level of this item (7 gems) gives 700hp itself + a ton of other benefits, but has a few issues - like dropping completely once mitigation shields have reached their max.
