Ruins of KunarkFind an iksar shaman to duel that has the archeology shield and cudgel quests completed, and ask for a dex buff and carnage click. If you have fungi tunic + staff or any form of regen, they can proc the shield on you and if you time it correctly you will both stay 80%+ HP the entire time. With the 215+ dex from buff+carnage (depending on your gear) you should have these done in no time.
Monk/Shaman Iksar box can do this with ease.

you have to use the hammer that you get for penance quest. the bone shatter proc is what makes them. good luck, takes forever.
