I have been hunting in the hole for a couple years (across 3 toons already) and have only ever seen this drop a couple of times. I would tag this as a very rare drop.
It dropped off of Urwenae the Cold in The Hole.

This item is good for when you pass 200 wisdom. At this point, you switch from going for wisdom items to mana items. After 200 wisdom, each additional point of wisdom only gives 1 point of mana (if I remember correctly). Well, ok, maybe that is an over-simplification, but you get the picture. If you want this item, I would recommend you just buy it, because by the time you can get it (with a raiding party), you will have access to better stuff. Still, there is a range of levels where this is a desirable item and well worth the 1k (lower end price) to 2k(overcharging price) that you will have to pay to get it.
P.S. Probably only Druids would want this because Shamans and Clerics can get similar items with higher A.C.

I believe the withered leather drops off of the undead further into the hole.
The loam encrusted stuff drops off of elementals, which you find right up front.

Probably the case...it's the first new item from The Hole in a long while, so most likely you are right on the money.

what does this drop off?
Erudite revenants randomly drop withered leather, the named one in the tower, dartain, drops the tunic.
