I can't find the correct "Tattered Note" for this, but it is given to you as one of your newbie items. When you turn it in to TeAnara, the Surefall Glade Druid Guildmaster, you get this tunic and the following faction changes:

Protectors of the Pine +25
QRG Protected Animals +15
Unkempt Druids -25
Guards of Qeynos +15
Dried Grass Tunic*
