The lost notebook mission also had one drop off a champion...
Most ppl on my server refer to this as 69.1. The Sandvarrow gemstone drops about once or twice every Hard mission I have been on. It is from some random mob but I see it all the time.

Had 2 drop within 5 mins of each other in "the lost gnome(hard)" mission. Came out of a drachnid infiltrator both times.
