Does this + to backstab damage stack if you have more than one item with this bonus?

Yes, it'll stack. Currently the max +backstab damage you can get, not including AAs, is 165. The cap is not known, if there is one.

+5 backstab damage? That's per hit, or is this a modifier of some sort which would = more damage than that?

Adds +5 damage every backstab. Say if you normally did 345 damage, wearing this you would do 350. Just like the Warded Mantle.

Also wanted to add that + backstab damage effects minimal damage which is non-mitigated, making it a valuable increase to DpS overall.

It's all right, I cleared up what I had posted too.
Another quick thing about backstab-- 140 is the minimum backstab without +BS gear or the DoDH Precise Strikes AA line. +125 is the new cap added in a past patch.

It is a 5pct modifier to the backstab skill. So for example if backstab skill was 200 the effective skill would be 210.
