Lost Dungeons of Norrath
This dropped in a Miragul (Everfrost) Dungeon, but our average level was about 61-62. This was definitely NOT a level 65 dungeon.
It was in a "Frozen table", which strangely was not traqpped nor locked.
FT1 aug. - Guildie Cleric won the roll and wears it to this day.

Actually, this particular augment does NOT appear to have a LORE tag.
Your right I didn't notice that I'll have to cheack in game to see if it has been changed before I update this list.

True but it can only go in one slot.

Unfortunately you cannot put more than one of the same augment on any particular piece of equipment. But nothing would stop you from putting the same aug on different pieces of equipment that go in the same slot. With this augment, if you had two range items you use situationally (different focus effects or what-not), you could slap it on each of them if you were lucky enough to get 2.
