Ruins of Kunark
After camping the 3 different locations to find this mob (upstairs dining cook area, basement stalls jester area, and across stalls area in skeleton finger/cracklin bones areas), I believe that you get the sapphire from the thick boned skeleton in the skeleton finger/cracklin bones area.
After numerous kills on the other locations, jester area gave only the thick skull, dining room gave only emerald, and after hours the skeleton finger TBS gave the sapphire.
Get ready for a long camp. Good Luck.

I looted this off of a Thick Boned skellie in Kurns basement.

Was camping the cook in the basement on the Combine server this weekend and picked this up, Do not know which mob it cam off of but if someone needs it I have it in my bank. Give Gimbles a tell in game 9-12PM EDT and it is yours
Iv been in kurns hunting thick boned skeletons for days now and I cant seem to get one of these. All I know is this gem CANT be a common drop. btw the thick boned skeleton at the basment apparently doesnt drop or at least iv not yet seen him drop it. Im hoping maybe I can get it off the ph thick boned for the cook.
Fight smarter not harder.
Been awhile, but iirc this one you need to get up top in room where cook spawns.
