250 Smithing does not gaurentee success. I have had 3 Smiths with a skill of 252 (Max)fail on the final combine for the gloves 3 seperate times.

All the Drops to make the Livestone drop in Plane of Earth.

The raw materials needed to make Livestone armor all drop in the Plane of Earth. (A few, like the Vial of Purified Mana, are player-made with store-bought ingredients.) All are tradable, so you can buy the raw materials (or sometimes, the finished subcombines) in the bazaar or from people who exp or raid in PoE.

Be warned, the raw materials sell for a pretty penny, and the subcombines and finished products for even more.

At maximum smithing skill (currently 250 + some skill enhancing item like a geerlok or trophy), expect a 40-60% success rate on the final combines.

250 smithing is only half the problem here. you also need tier 4 flags.

The person with the 250 smithing doesn't HAVE to have tier 4 flags. I don't and I have made 2 livestone plate breastplates, from materials purchased in the bazaar.
