I finally looted the runed cowl after staying at the Sage camp about 4 hours a night for 3 days and most of a Saturday. I had a 45 necromancer and a 48 druid there most of the time but friends would drop in occasionally to keep me company. I stayed at the entrance to the Sage room - it's a relatively safe spot right under the spiderweb. From there I could pull the Sage room and Executioner room with ease. When I was more confident I started pulling the Savant room and the Cavalier room. It was pretty good exp at those levels. Almost everything turns green at level 51.
My experience with the sage was that he spawned about 1 in every 4 or 5 spawns on average. I killed him about 12 times total to get the cowl. The braided cinch cord always droped except when the sage had the cowl. I nuked both the sage and the scribe to kill them. The scribe would often charm the necro and bug his pet so that he would have to log to make a new pet. A level 48 can pull the sage room without aggroing the hidden wizzy's but a level 47 sometimes got one of the wizzy's with the pull.
Once the room was broken, I would park the pet in the secret room before the wizzys spawned. Both the druid and the necro would target the pet. When the wizzy frog's spawned they would aggro the pet. The necro then used /assist to target and lifetapped while the druid would heal the pet if needed. This seemed to work well.

First 10 hours he spawned once, with the cords. Second time I camped him for 10 hours he spawned twice both with cords. The runed cowl is pretty rare and my enchanter is getting pissed cuz he wants this damn helmet. GAH /rude VI
Yesterday I was there with my 45 mage and grouped up with a 44 necro, and the Sage popped three times in a row, and he had the cowl all three times. The Urd Wizard PH popped the next two pops after than, and then I had to log.
So I think it relies more on the sage to actually pop in the first place, but the cowl is the rare of his drops and the braided cinch cord is the common drop.
