She does sell it! You just have to make sure you uncheck the box "Show only items I can use". I would have saved myself a bunch of time if I had just scrolled a little bit further down the message board. :-P
I just did a combine breaking down small brick of ore into small piece of ore and my smithing chisel went poof. It wasn't on the ground or in my inventory or in my weapon slot. I wouldn't be able to put in my weapon slot anyway, since I am wrong class to be able to wield it.

Currently (as of the 2-26-2018 patch) there is a known bug where return on success/failure tradeskill items are not being given back to players. I believe they have it fixed internally but until the next patch it is an ongoing issue.
NOTE FOR THE FUTURE: If you are reading this past April of 2018 this bug has likely been fixed by now.
I have experienced this with doing tarnished weapons (sharpening stone plus rusty weapon). If you fail you are suppose to get your rusty weapon back and it eats the stone. But the last couple days it takes both the stone and the rusty weapon on fails.
If you cannot find this at the vendors, make sure you uncheck the little box at the bottom of the vendor screen for items that you can't use. This is technically a weapon, so if you are not able to equip the weapon, it will not show up as available for you to purchase. It only took me like 2 hours of porting all over the world to figure this out!
Confirmed Tallis Paerk at the above location sells them, he's the guy that sells the ore needed for skilling up in smithing so you probably know him too well already!
The sequence is a rip-off but...
block + chisel = 1 large (gip!) large + chisel = 1 small (huh I'm wasting most of it!) small + chisel = 2 pieces (better but...)
I don't know what the trivial on this is on the conversion, it was triv with smithing of 175 but even with wisdom of 260 I failed on 4 conversions out of 15.
These chisels have always been available from the same vendors that have the forging hammer mentioned above. There just wasn't much reason to include it in the item database until they decided to be really completist about it.
Note: By "always", I don't truly mean since EQ first went live. But certainly long before now, probably at least since sometime around Kunark release, given its use and the forging hammer's.
MAYBE...just MAYBE...this will be an item where you can take large bricks and blocks of ore and knock them down to things like small bricks and small peices of ore! Kinda like you do with the skinning knife with tailoring...Yeah, that's it! I HOPE...;)

If it suddenly showed up at merchants, and seems useless, it's probably for a quest that requires smithing. Like the forging hammer (not smithy hammer). Or maybe they will finally add some new smithing recipes that are similar to the new tailoring. /em crosses his fingers.
