Rain of Fear
For anyone currently searching for this item, it is rewarded upon completion of the achievement Lesser Fearspeaker.
With the release of Tier 3 (April '13) and Tier 4 (July '13) additional achievements were added and existing achievements were modified resulting in various Lesser and Greater achievements. Prior to that, this item was rewarded by Fearspeaker. The requirements have not changed. Still need to complete all T1 and T2 content.

This is the reward for the Rain of Fear achievement Fearspeaker which is gained when completing Champion of Rain of Fear (all Mercenary and Partisan tasks) and Paragon of Rain of Fear (all Hero group missions).
You also get the suffix "the Fearspeaker".
