Lore:
This complex device allows you to modify the appearance of some guild hall assistants.

Usage Instructions:
The following characteristics can be modified, please note that not all of these characteristics will work on all appearances. Not even the most advanced gnomish scientists have been able to grow a beard on a wood-elf yet...

To change a characteristic of an assistant, hold the device and activate it without your focus on any target. This will allow you to cycle through the characteristics that can be modified.

Once you have selected the characteristic you wish to modify, focus your attention on a target and activate the device.

This item is the result of a quest.

"Spell Effect: This complex device allows you to modify the appearance of some guild hall assistants"

Can anyone clarify exactly what a "guild hall assistant" is and does that mean I cant use this to change appearances in my house but only in the guild hall?

Quick summary of how to get this:

Obtain the item "The Thornless Rose" from Behroe Dlexon in South Qeynos by saying "what ballad" to him, then turn it in to Arwik Nimbletune in Sunrise Hills.

This ties in to the older quest called "Aenia and Behroe".

For more details, see people's posts under Arwik Nimbletune, and perhaps the upcoming quest entry.
